Alzheimer’s/Dementia Caregiver Program
Our Alzheimer’s/Dementia caregiver program and substitute caregiver to care for the needs of their loved one.
This Medicaid program was developed to provide an option for individuals diagnosed with Alzheimer's or a related disorder who wish to remain living at home.
This program is unique in that it requires both the primary caregiver and client to reside in the same home, whether it be the private home of the primary caregiver or the client. Again, the primary caregiver may be a non-family member, a family member or a legal guardian, which would include the spouse.
Requirements:
Client must be 21 years of age and over and
Diagnosed with Alzheimer's or a related disorder by a physician licensed in the State of Missouri.
The client and caregiver must reside full time in the same household.
Established backup plan: A qualified substitute caregiver familiar with the client’s needs, employed by our agency and available to provide services at times when the primary caregiver is not available.
Be in active Medicaid status.
WE PAY YOU to provide:
*Activities of daily living (ADLs) such as grooming and dressing
*Medication oversight (to the extent permitted under State law)
*Escorting to necessary appointments such as doctor appointments and therapeutic community activities
The goal is to provide necessary care while fostering the client’s independence in a home environment. This goal is reached through a cooperative relationship between the client (or client’s legal guardian), caregivers, and our agency.
Both the primary and substitute caregivers receive initial and ongoing support from our agency in order to provide high quality care. No need to worry, we provide primary and substitute caregivers with a minimum of 8 hours annual training, during monthly home visits or through secure electronic communication methods, dependent on the client’s needs.
